Local partners are increasingly working together to simplify and join up their assessment approaches so that clients get a really thorough initial assessment, agree their initial steps and achieve their initial referral successfully.
This initial assessment may be revisited and refined but clients will not experience re-assessment every time they go to a new service.
During this stage each client will gain an ‘account manager’ whose job is to help clients find their way through a complex array of service.
The initial task will be to agree the first few steps of their journey and make the appropriate referrals.
